I understand although the Management of Rainbow Omega Inc., makes every effort to accommodate individual preferences, business needs may at times make the following conditions necessary: overtime, rotating work schedules, a work schedule other than Monday through Friday and/or cancellation of scheduled leave, etc.

I understand if offered employment with Rainbow Omega Inc., (ROI) my employment will be contingent upon the negative results of pre-employment drug screen. Upon receipt of a confirmed positive result, my employment offer will be withdrawn and I will no longer be considered for employment. If hired, I consent to submit to periodic random drug and alcohol testing as well as alcohol and drug testing for reasonable suspicion or for cause. I also understand during the hiring process or while employed with ROI, a refusal to submit to drug and alcohol testing, tampering or altering the test sample in any way, is considered a “positive” result. I further understand, such an occurrence will result in my employment offer being withdrawn and I will no longer be considered for employment with ROI. If employed, such an occurrence will result in immediate termination of my employment with ROI.
